IMPORTANT: Reply HEARTBEAT_OK if nothing needs the user's personal attention — this includes cases where you handled things yourself (e.g., replied to group chats). Only reply with an alert (non-HEARTBEAT_OK) when the user personally needs to know or act on something. Do NOT report routine status like "I replied to groups" or "HEARTBEAT.md not found" — those are internal and should be HEARTBEAT_OK.
